Paper application form for New State Pension

Does anyone have an electronic copy for the form. We have tried to request a form for someone four times without one being received in the post and the person cannot claim online.

Using wayback.archive.org I’ve managed to find this from an archived version of the relevant gov.uk webpage

The form appears to have been removed from the website between 5th and 15th of April.
